Your initial appointment will consist of an interview with one of our dental hygienists to discuss your concerns followed by an examination, cleaning of your teeth (unless a finding of periodontal disease is noted) and an explanation of findings by Dr. Blanton and the hygienist and recommendations for any necessary treatment as well as options and costs.An extensive medical history or complex dental issues may require further evaluation and a second appointment to thoroughly review conditions and treatment options.
Please assist us by providing the following information at the time of your initial appointment:
- X-rays if you have them and they are less than one year old
- The name and address of your previous dentist so we can obtain any pertinent information or x-rays
- A list of medications you are presently taking
- If you have dental insurance, please bring your insurance card with you. This will save time and allow us to help you process any claims.
If your former dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office if possible. If additional films are necessary, they can be taken at our facility.
IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the initial office visit.
Please alert the office if you have a medical condition that may be of concern i.e. a history of mitral valve prolapse, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc. or if you are on any medication i.e. heart medications, aspirin, anticoagulant therapy, etc.
